  • Before you begin, save and close all documents listed below. You will not be able to upload documents left open on your computer.
  • After you have completed the application, select "Submit"
  • After submission, Arts Midwest staff will review applications for theater company eligibility, project requirement fulfillment, and the inclusion of all application components. If we have questions, we will contact you. If not, you will receive an email acknowledging the receipt of your application along with a PDF of your complete application as it will be forwarded to the advisory panel.

The strategy for reaching middle or high schools

The advisory panel will review the following components for this selection criterion, including evidence of the company's minimum of three years' experience providing performances and educational activities to middle and/or high schools, feasibility of reaching 10 or more schools, the inclusion of underserved schools, and (if applicable) plans to increase the number of schools or reach new schools.

By submitting this application, I certify on behalf of my theater company that the information provided in this application is true to the best of my knowledge. In addition, my theater company gives Arts Midwest permission to copy or digitally convert our work sample and disseminate it for the purpose of the panel review.

On December 26, 2013 OMB issued 2 CFR 200: Uniform  Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards (Uniform Guidance) which combined and streamlined eight previous OMB Circulars that governed the management of federal assistance awards. The Uniform Guidance eliminates duplicative and conflicting guidance in these documents. 
After Submission of the Application Form
After the February 2 deadline, Arts Midwest staff will review applications for theater company eligibility, project requirement fulfillment, and the inclusion of all application components. If the staff has questions about your application, they will contact you. If there are no questions concerning your application, you will receive an email acknowledging the receipt of your application along with a PDF of your complete application as it will be forwarded to the advisory panel.
