    2017-2018 Arts Midwest Touring Fund Application 


    Before completing the application, please read the program guidelines and visit the application contents page at the Arts Midwest Website. Only one application per organization may be sumbitted.


    Application & Submission Instructions

    • The application form is one page
    • You will not be able to save and return to the application form. 
    • Before you begin, save and close all documents to be uploaded. You will not be able to upload documents left open on your computer.
    • After you have completed the application, select "Submit"
    • You will be taken a Summary page of all your answers.
    • Please review your responses and print a copy for your files.
    • When finished your review, hit "Confirm" to complete your application.
    • Following staff review, you will receive a confirmation notice with details of approved underserved audience and star project components.

    Provide information for the artist/ensemble (NOT its manager/agent) in this section. This information is for internal use; only the artist name and state may be publicized.

     
    Determining an artist/ensemble's state of residence: 
    • Select the state where the artist is located. 
    • For an ensemble of artists located in more than one state, select the state in which the ensemble’s business is conducted. 
    • Remember, the artist MAY NOT be located in the same state as the applicant.

    Upload either a letter of intent or draft contract between your organization and artist/ensemble. No signatures are required at this time.

    • Names of both parties (presenting organization and artist/ensemble and/or agent) 
    • Date(s) of the engagement 
    • Artist/Ensemble's contracted fee (must be at least $2,500)
    • If confirmed, details of public performances and community engagement activities.

    On December 26, 2013 OMB issued 2 CFR 200: Uniform  Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards (Uniform Guidance) which combined and streamlined eight previous OMB Circulars that governed the management of federal assistance awards. The Uniform Guidance eliminates duplicative and conflicting guidance in these documents.
